Pesticide Notification & Sunscreen Consent

Please read each section carefully and provide your printed name, signature, and date.

Pesticide Notification

As part of Troy Babes in Toyland's pest management program, pesticides are occasionally applied (either after hours or on the weekends). You have the right to be informed prior to any pesticide application made to the interior/exterior of the building. Rose Exterminator comes out the third Thursday of each month. In the rare occurrence that anything is treated, there will be a notice posted by Monday morning on the front door of the center as well as in our newsletter. In certain emergencies, pesticides may be applied without prior notice; you will be provided with a notice following any such application by the posted note hanging on the door as well as on our website.

Sunscreen & Water Sprinkler Play

I provide consent for Troy Babes in Toyland to apply sunscreen on my child when he/she is exposed to the sun during enrollment at Troy Babes in Toyland. I understand that the only sunscreen that will be used on my child is the one that I provide. I also provide consent for my child to participate in sprinkler play at Troy Babes in Toyland. I understand that I must provide a bathing suit and towel.

ENROLLMENT AGREEMENT

Section 1: Parent/Guardian Information

Section 2: Tuition and Fees

Please initial each item to acknowledge you have read and understood it.

REGISTRATION FEE: I understand that an annual, non-refundable Registration Fee of $100.00/$150.00 for a family shall be paid at the time of enrollment for my child.

DEPOSIT: I understand that upon enrolling my child I am required to pay a non-refundable one-week tuition deposit. This deposit will be held for the last week of care. I understand that this deposit secures a space for my child for up to one month. If my child does not attend within one month, his/her spot will forfeit and a new deposit will be required.

TUITION FEE: I understand that rates are subject to change with reasonable notice. We agree to pay the full fixed weekly rate regardless of absences.

PAYMENT OF TUITION: Weekly tuition is due the Monday of the week services are rendered. If paying monthly, tuition is due on or before the first of every month.

LATE OR UNPAID TUITION: If payment is not received by Monday, a $30 late fee will be assessed. Any unpaid tuition fees may be sent to a third-party collection agency.

AGENCY (DHS) REIMBURSEMENT: I understand that I am solely responsible for any tuition payment and late fees in excess of any agency or third-party reimbursement. I am also solely responsible for promptly communicating ANY changes in my status that would affect my agency payments.

DISCOUNTS: I understand that if my child attends full-time, a fifteen percent (15%) discount is offered for an additional child from my immediate family who enrolls. The discount is applied to the lowest tuition rate.

RETURNED CHECKS: A processing fee of $35 will be charged for all returned checks. Non-sufficient funds checks will be automatically resubmitted electronically up to three times. If more than two checks are returned within six months, payment by alternate method will be required for the next six months.

ABSENCES/MISSED DAYS/VACATIONS: Your child's space is guaranteed; refunds or credits will not be granted for absences. Each full-time child enrolled will receive two vacation weeks per school year (September–September) at half the normal tuition rate.

WITHDRAWAL FROM PROGRAM: I must provide a two (2) week written notice of withdrawal. If not provided, I cannot use my Deposit. When my child is withdrawn, re-admission is based upon space availability. A new Registration Fee and Deposit will be required for re-enrollment. All fees are non-refundable.

Section 3: Daily Procedures and Policies

DAILY DROP-OFF & PICK-UP POLICY: Parents are required to sign their child(ren) in and out at the computer near the entrance using their 8-digit PIN. Children will not be released to anyone other than those authorized on the Emergency Information Card. A valid picture I.D. will be requested from anyone other than the parents.

SICK CHILD POLICY: I understand that I will be notified by phone should my child become ill and that I will pick up my child promptly. If my child is exposed to or contracts a contagious disease, I agree to notify the school and understand my child will be re-admitted according to the Babes in Toyland Illness Policy.

ILLNESS POLICY: A child may not attend if he/she has a fever of 101.0° or greater, diarrhea, vomiting, serious cold symptoms, skin rashes, or Hand Foot and Mouth disease. Please refrain from administering fever-reducing medication prior to bringing children to school. Any child sent home may not return for 24 hours.

DISCIPLINE POLICY: Troy Babes in Toyland is committed to the HighScope Conflict Resolution model: (1) Approach calmly, stopping hurtful action. (2) Acknowledge feelings. (3) Gather information. (4) Restate the problem. (5) Ask for solutions and choose one together. (6) Give follow-up support.

BITING POLICY: Biting is a natural phenomenon among infants, toddlers and preschoolers. Our policy: (A) Care for the child who was bitten first. (B) Re-direct the child who bit. (C) Work with the child and examine the classroom. (D) Shadow the biter and record instances. (E) Conference with parents. Both sets of parents will be advised via Accident Reports.

MEAL POLICY: Troy Babes in Toyland provides healthy, well-balanced meals and snacks including whole grains and fresh fruits. Two nutritious snacks and hot lunch are provided daily at no additional cost. Nursery & Toddler parents agree to provide food for their infant/toddler per the stated guidelines. A monthly menu will be posted on the parent board and website.

SAFE INFANT SLEEP POLICY: Troy Babes in Toyland follows the recommendations of the American Academy of Pediatrics (AAP) and the Consumer Product Safety Commission to provide a safe sleep environment and reduce the risk of SIDS.

SLEEP POSITION: Infants will be placed flat on their backs to sleep every time unless there is a physician-assigned sleep position medical waiver on file. Devices such as wedges or infant positioners will not be used. Pacifiers will be offered at sleep time and cleaned between each use.

SLEEP ENVIRONMENT: Safety-approved cribs with firm mattresses will be used. Only one infant per crib. The crib will be free from loose bedding, toys and soft objects. Room temperature will be kept comfortable for a lightly clothed adult. Smoking is prohibited in or near Troy Babes in Toyland.

SUPERVISION: Sleeping infants will be within sight and hearing of staff at all times. A staff member will visibly check on sleeping infants frequently. Awake infants will have supervised tummy time.

TRAINING: All staff, substitute staff and volunteers will be trained on safe sleep policies and practices before caring for infants, and reviewed annually. Documentation of training will be kept in each individual's file.

Section 4: Consents

SUNSCREEN CONSENT: I provide consent for Troy Babes in Toyland to apply sunscreen on my child when exposed to the sun. I understand that only the sunscreen I provide will be used, and it will be in lotion form.

ASSESSMENT CONSENT: I understand that Troy Babes in Toyland completes developmental assessments using Ages & Stages Questionnaires (ASQ-3), ASQ-SE, and the HighScope C.O.R. I also understand that I must complete the online ASQ-3 & ASQ-SE annually during registration time.

VIDEO SURVEILLANCE: I hereby give my consent to video surveillance at any time the center may choose. Cameras are positioned in appropriate places within the building to promote safety and security. The only exception is private areas of restrooms.

PHOTO RELEASE: Troy Babes in Toyland uses a center camera to photograph children during projects, parties, play and classroom interactions. Photos may be posted in classrooms, hallways, the Parent Portal, and occasionally used in brochures, the website, and Facebook. Names and personal information are never disclosed.

Section 5: Licensing Notebook

I understand that Troy Babes in Toyland maintains a licensing notebook of all licensing inspection reports, special investigation reports and all related corrective action plans.

I understand that this notebook will be available to parents for review during regular business hours.

I understand that licensing inspection and special investigation reports from the past two years are available on the Bureau of Children and Adult Licensing website at www.michigan.gov/michildcare.

Section 6: Pesticide Notification Request

Section 7: Volunteer Statement

PARENT INVOLVEMENT: Babes in Toyland welcomes and encourages volunteers. As a volunteer, a staff member will supervise you at all times. You will not be left alone or assigned toileting/diapering duties. Duties include assisting with art activities, special functions, group activities, reading stories, music and movement, and outdoor play.

Have you ever been convicted of a felony involving harm or threatened harm to an individual?

Have you ever been convicted of a crime involving abuse and neglect of a child?

Abuse/Neglect Statement:

  • You are aware that abuse and neglect of children is against the law.
  • You have been informed of the center's policies on child abuse and neglect.
  • You know that caregivers are mandated by law to report abuse and neglect.

Section 8: Health Care Service Plan

Please initial each item to acknowledge you have read and understood it.

HEALTH SURVEILLANCE REPORTING: Babes in Toyland will report observed changes in the child's health or any accidents to the parent. Daily sheets are completed for children under 2½ years. Accident reports must be thoroughly completed. A signed medicine form is required from the parent for any medication administered.

FOOD, FORMULA & MILK: Caregivers wash hands prior to each feeding. Bottles are heated by submerging in heated water. Children's food and bottles are labeled with first and last name, contents, amount and date. All children at least 1 year old will be served whole milk unless otherwise directed. Lunches are served family style.

DIAPERING, RUNNY NOSES AND HAND WASHING: Universal Precautions are followed. Hand washing before and after diapering is required. Diaper table is washed and sanitized after each use using the 3-step method. Children's diapers are changed every 2 hours and when soiled or wet. Children's hands are washed with soap and running water after each diaper change.

EQUIPMENT: SAFE AND SANITARY: Toys are cleaned using the 3-step method (soapy water, rinse water, bleach water). Stuffed toys and dress-up clothes are laundered weekly in hot water. Carpets are vacuumed several times daily and professionally cleaned every six weeks.

REST AND NAP TIME: Infants under 12 months sleep in cribs with tight-fitting sheets. No blankets for children under 12 months; sleep sacks are used. Children 12 months and older sleep on cots or mats. Parents provide bedding and take home each Friday to launder. Children are provided the opportunity to rest and are never forced to sleep.

STAFF TRAINING PLAN: Babes in Toyland participates in the C.D.A. Scholarship program and encourages staff to attend approved quality training through 4C's & Great Start to Quality. Staff receive a minimum of 24 clock hours of professional development. All staff are required to obtain annual Blood Borne Pathogen, CPR and First Aid training.

BITING: Biting is a developmentally appropriate behavior for children in the Nursery, Toddler and Young Pre-S rooms. Staff will not punish children for biting; they will encourage the child who bit to help care for the child who was bitten and redirect children to different activities. Parents will be notified by incident report.

FIRE, TORNADO, DISASTER DRILLS AND EVACUATION: Babes in Toyland conducts fire, tornado, and emergency evacuation drills without prior notice to parents, staff or children. During a drill or actual emergency, parents may not sign children in or out until the drill is complete and children have returned to the building.

ALTERNATE SAFE LOCATION: Should the administration deem it necessary to evacuate the building, we will follow the evacuation plan and head to the predetermined safe zone. Parents will be notified via phone by office staff.

HEALTH CARE PLAN ACKNOWLEDGMENT: I have read and understand the Babes in Toyland Health Care Plan and Staff Training Plan and will comply with all guidelines and rules set forth.

I have received information on the ASQ-3 & ASQ/SE which I understand I must complete online at a minimum once annually.

I have received a Health Appraisal form, which must be filled out by a doctor and returned within two weeks of my child's start date. If I do not choose to vaccinate my child, I am required to sign a waiver form and return it to the office.

I have received an Emergency Information Card. I understand it must be filled out annually and COMPLETELY. I agree to update the card immediately if I move, change employers, or change phone numbers. My child will not be released to anyone not listed on the Emergency Information Card.
